If you'd asked this question five or six years ago, you'd probably have got a very different answer.
Back then, tear trough filler was being offered everywhere. If you looked tired, had dark circles or noticed hollows under your eyes, chances are someone suggested filler.
Fast forward to today and it's become one of the most debated treatments in aesthetics. Some practitioners no longer offer it at all. Social media is full of horror stories, videos about filler migration and people warning others never to have it done.
So, what changed? Has tear trough filler suddenly become unsafe?
Spoiler! No.
What has changed is our understanding of who it actually works well for. We've become much better at recognising that this isn't a treatment for everyone, and I think that's a really positive step for the industry.

What Is Tear Trough Filler and Why Has It Become So Controversial?
Tear trough filler is designed to soften the hollow that runs from the inner corner of the eye down towards the cheek.
When that hollow becomes deeper with age, or is naturally prominent, it creates a shadow that can make you look permanently tired, even when you've had a full night's sleep.
Nobody has ever walked into my clinic and said, "Gem, I really want everyone to know I've had tear trough filler." They just want to stop looking bloody exhausted.
The problem is that, for a long time, tear trough filler became the answer to almost every under eye concern.
Dark circles? Filler.
Eye bags? Filler.
Tired eyes? Filler.
The reality is much more complicated than that.
Is Tear Trough Filler Safe?
Yes, when it's carried out by an experienced practitioner on the right patient.
But it also happens to be one of the most technically demanding filler treatments we perform.
The skin under the eyes is incredibly thin, the anatomy is complex, and this area doesn't forgive poor technique or poor patient selection.
That's one of the reasons you'll hear experienced practitioners speak about tear trough filler differently today than they did a few years ago.
The treatment itself hasn't changed. Our standards have.
We're much more selective about who we treat because we've learnt that saying "no" often leads to better long term results than trying to make someone fit a treatment they were never suited to in the first place.

Who Is a Good Candidate for Tear Trough Filler?
This is probably the most important question of all.
The best candidates are people with genuine volume loss under the eyes that creates a hollow or shadow.
If that's what's making you look tired, tear trough filler can produce beautifully subtle results.
But not everyone who dislikes their under eyes actually has a tear trough problem.
Sometimes the issue is:
prominent eye bags
skin laxity
pigmentation
fluid retention
loss of cheek support
thinning skin
And filler only really addresses one of those.
That's why every consultation starts with understanding why you're seeing those changes, rather than jumping straight to treatment.
Can Tear Trough Filler Make Eye Bags Worse?
Yes, it can.
And this is one of the reasons the treatment has developed such a poor reputation.
If someone already has prominent eye bags, adding filler underneath them can sometimes make the area appear heavier or puffier instead of fresher.
The same can happen if too much filler is used or if the treatment is repeated unnecessarily over time.
This is where some of the horror stories online come from.
Not because tear trough filler is a bad treatment, but because it was carried out on someone who wasn't the right candidate, or more filler was added when the answer was never filler in the first place.

Why Some Practitioners No Longer Offer Tear Trough Filler
One of the biggest changes I've seen in aesthetics over the last few years is that experienced practitioners have become much more comfortable saying no.
Personally, I think that's a good thing.
Five years ago, the industry was very different. Bigger lips were fashionable. Pillow face was far more common than it is today. We moved into the era of "Instagram face", where everyone seemed to be chasing the same exaggerated cheeks, lips and chin.
Thankfully, we're moving away from that now.
The focus has shifted from changing faces to respecting them.
That means practitioners are looking at the whole face instead of treating individual features in isolation.
Sometimes the tired appearance under the eyes isn't caused by the tear trough at all. Sometimes improving cheek support creates a much better result. Sometimes skin quality is the priority. Sometimes regenerative treatments are more appropriate.
And sometimes, the best treatment is simply explaining why filler isn't the answer.
I'd much rather disappoint someone during a consultation than disappoint them every time they look in the mirror afterwards!

What Are the Best Alternatives to Tear Trough Filler?
One of the biggest misconceptions is that filler is the only way to improve the under eye area.
In reality, that depends entirely on what's causing the concern.
For some people, restoring support to the mid face with cheek filler indirectly improves the tear trough by reducing the shadow beneath the eye.
Others may benefit more from treatments that improve skin quality, such as polynucleotides, which encourage collagen production and help strengthen the delicate skin around the eyes over time.
Sometimes a combination of treatments achieves the most natural result, and sometimes no injectable treatment is the right answer at all.
The important thing isn't choosing the most popular treatment. It's choosing the right treatment for your face.
